Membership Application Form

To become a member of IPMS Canada, simply print out this application form (click here for an MS Word format document), fill in the information, and mail it to the address below, along with the appropriate membership fee.

MEMBERSHIP FEES
Canada - CAD $24.00 per year
USA - USD $24.00 per year
Other Countries - USD $29.00 per year

NOTE: The only forms of payment we can currently accept are: 

  • cheques drawn on Canadian or US banks, 
  • Canada Post money orders, 
  • International money orders (USPS money orders are not acceptable, as they are only negotiable in the USA), or 
  • cash (but we don't recommend this method, and can't be held responsible for cash lost in the mail)
